Non-Job Action Background Check Request

If an employee needs to begin new duties that make a background check reasonable, please use our Non-Job Action Background Check Web Form to notify us of your request. This could include duties related to cash-handling, finances, driving, working with minors, or those related to an interim appointment that includes broader, risk-based duties. The form will ask you to provide your name (HR Representative), department OUC, and contact information, along with the employee’s, name, email, EMPL ID, and current job. Please mark ALL checks that are needed for your employee. If you are not sure which checks are needed please leave that section blank, and the BCP will help make that assessment. In addition, please detail the reason for your request. This information will be submitted directly to the Background Check Program for processing. Remember, this tool is valid for existing employees only.
